Abstract

In this study, we show a big picture of accident investigations in the oil and gas industry through the analysis sixteen accident investigation reports. Accidents were represented using AcciMap method, where each of the contributing factors identified was
plotted in its socio-technical level related to the system. Afterwards, the contributing factors were classified into generic categories allowing the abstraction of data and the analysis of the frequency distribution between the different levels. Of the 589 contributing factors identified, 55% appear at operational levels, 37% at company levels and 8% at regulatory and
government levels. Our results indicate that the independence of the investigation boards and the methods adopted are determinants to the systemic approach level and learning
achievements in any investigation.
